ARCTIC GOLD SCHIPPERKES

PUPPY QUESTIONAIRE

The following questions are being asked of you so that the right puppy and the right placement of each puppy is made by bringing the various requirements of both the puppy and his/her new owner(s) into perspective, BEFORE a choice is made.   I hope you will agree that the animal's welfare must be my foremost consideration in considering a placement.   Please answer the following questions as completely as you can, and return this form.   You may print it, complete it and drop it in the snail mail, or copy/paste into an email and send it back.

Name:

How did you hear about the Schipperke?

What reference materials have you read?

Have you met a Schipperke in person? Whose?

How long have you lived at this address?

Type of dwelling:        House        Condo        Apartment    Mobile home

If renting, please give name, address & phone number of landlord:

Who will be the primary care giver?

Where will the puppy be kept during the night?

Where will the puppy be kept during the day?

Is anyone home during the day?

How many hours on the average will the puppy be left alone?

Will there be someone available to feed and exercise the puppy during the day?

Have you ever owned a dog before? What breed?

How long did your last pet live? What were the circumstances of its' death?

Do you have any other animals?

Do you have any children? If yes, what are their ages.

Are you interested in a male or female puppy? Why?

Are you most interested in a Puppy (8-12 weeks) or would a young adult (6-18 mo), or retired show dog (3+ years) interest you? Occasionally we will keep a show/breeding prospect and for one reason or another does not work out and becomes available to a good home.  These dogs have had manners/leash training, house training, crate trained and likely started obedience training as well. The reasons we would be parting with these individuals vary from a wide variety of structural "not-quite-good-enough-to show/breed" reasons including:  poor topline, over angulated rear, low tail set, ear set, etc. and are never personality/disposition flaws.

What do you expect the activity level of your dog to be:
very high    high    moderate    below average   (circle one)

How will the puppy be confined when he is outside?

Do you have a fenced yard or suitable pen?

Have you ever house trained a dog before?

Are there training classes nearby? What is the predominant training method and do they offer Puppy Kindergarten?

Which activities or competitions interested you?

Are you interested in showing the dog in the conformation ring?    YES    NO

Are you interested in competition in performance events such as obedience, agility, search and rescue, tracking, etc.?    YES    NO
If yes, have you shown a dog to any performance titles?

Do you intend to breed the dog?    YES    NO

Additional information that you'd like to offer that will help us in puppy placement.
